Every organization has forms, and we are no exception. download them, print them, enjoy them! It is as easy as "right click" on a form, and then save locally for printing.

• The Release of Liability (2011 - 2012 season) For those 18 years of age and older

• The Juniors Release of Liability (2011 - 2012 season) for those 17 years young and younger

• 2012 Spring Membership Form - Forms should be sent to secretary@curldc.org.

• League Descriptions (September 2011) as well as the League Sign-up online (September 2011)

• Reimbursement Request (generic form, not year specific) in either .pdf or .xls format
